The Duke Endowment Healthy People, Healthy Carolinas (HPHC) Grant
Collaborative work with Atrium Health Wake Forest Baptist, Moses Cone Hospital, Foundation for a Healthy High Point, Guilford County Cooperative Extension, and additional community partners.

The work on the HPHC grant began in the spring of 2022 and continues to this day. The project has brought many partners together to improve nutrition and physical activity in Guilford County.
This has been a massive project involving Atrium Wake Forest High Point Medical Center, Moses Cone Hospital, Guilford County Public Health, Foundation for a Healthy High Point, Cone Health Foundation, NC Cooperative Extension-Guilford County, GHPFA, and other collaborative partners.
This project required extensive advocacy, equity, diversity, inclusion, and community engagement training. We have also had to navigate complex administrative systems for the project administration.
The Duke Endowment awarded the HPHC grant to the Guilford Coalition. This is a five-year grant for a total of $750,000.
This is a wonderful example of the power of collaboration and community engagement. We look forward to hiring a Project Director to lead this project and the community engagement that will be involved.
